OHLAY BRAND EVENTS WAIVER & RELEASE FORM PARTICIPANT RELEASE OF LIABILITY --- READ BEFORE SIGNING In consideration for being allowed to participate (ride) in any way at OHLAY Brand Events, its related events, and activities, the undersigned acknowledges, appreciates, and agrees that: 1. The risk of injury from the activities involved in these programs is significant, including the potential for permanent disability and loss, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury to me does exist; and, 2. I knowingly and freely assume all such risk, both known and unknown, even if arising from the negligence of the releasees (as defined in paragraph 4 below) or others, and assume full responsibility for my participation; and, 3. PREPARE TO RIDE WAIVER & SAFETY INFORMATION Freestyle scooter riding is dangerous. Scootering presents a significant inherent risk of serious physical injury, including paralysis, disability or death. When you ride at OHLAY Brand Events, you voluntarily assume all risks associated with riding. Event participants (riders) must complete a waiver. Filling out the waiver online before the event will save you time the day of the event. For those riders under the age of 18, a parent or legal guardian* must complete a waiver on the riders behalf.
